OUR HOSPICE TEAM

We are proud of our specially trained interdisciplinary team, who will help take care of you.
The team includes:

  • Your Attending Physician – actively participates in the plan of care and remains your primary physician.
  • Our Hospice Medical Director – oversees and leads the team in developing a plan of care and consults with your primary physician.
  • A Registered Nurse – assesses and manages pain and other symptoms related to the disease. The Nurse coordinates services and care with the patient’s physician and primary caregivers, and also makes regular visits teaching caregivers how to care for family members and provide comfort care.
  • A Social Worker – assesses the patient and family’s emotional, social, and financial needs; implements the plan of care to meet the patient and family’s needs; and provides direct counseling and bereavement support.
  • Trained Volunteers – they are specially trained to provide companionship and support to the patient and family, provide respite for the caregiver and support during bereavement.
  • Spiritual Counselor – provides support and comfort directly or contacts local clergy at the request of patient/family.
  • Bereavement Counselor – provides support during Hospice care and for 13 months after death. Services can range from crisis intervention, information, and intervention for children to private and group grief support and family/ individual counseling.
  • Dietician, Physical Therapist, Occupational Therapist, Speech Therapist, and other Therapists – who are available by consultation as needs are identified.
  • Multi-Lingual – We are proud of our specially trained, multi-lingual interdisciplinary team, who will help take care of you.
  • Bereavement Support – offers compassionate care and guidance to individuals and families navigating grief. Providing a safe space for emotional processing, they offer tailored support to help cope with loss and build resilience.
